Before you begin. Filling out this questionnaire does not make you a client of Montague Law PLLC (the Firm) and does not create an attorney-client relationship. Until then, the Firm has no duty to act on anything you submit, including any deadline. Submitting answers does not prevent the Firm from representing other clients, including clients whose marks or interests may conflict with yours, except where Kentucky's ethics rules provide otherwise. We keep what you submit confidential and use it only to handle your sign-up and for conflicts checks. If you do not finish, a draft saved on our systems is deleted 14 days after you last saved it. Nothing is filed automatically. Please do not include passwords, financial account numbers, or anything beyond what the questions ask.

We run the conflicts check every law firm must run, usually within 3 business days. If anything prevents us from going ahead, we will tell you right away.
